CHIROPRACTIC & MASSAGE THERAPY
As a courtesy to our patients, we will prepare
and submit your insurance forms for direct
reimbursement if we accept assignment of
your insurance benefits. You may have a
deductible, and are required by law to pay
your co-payment. We are participating
providers in the Blue Shield PPO and United
Health Care. Out of Network benefits will
apply to all other insurance.
Personal injury cases can be billed directly
through the insured’s medical payment portion
of your major medical automobile policy.
MEDICAL & NATUROPATHIC
A super-bill with procedure and diagnostic
codes will be prepared for you to submit to
your insurance company for reimbursement.

Tiburon Wellness Center is located in the old
fishing town of Tiburon California,
in the beautiful Marin County Bay Area.
From the North
Take Highway 101 South to the Tiburon Exit.
Go East over the highway
& continue straight on Tiburon Boulevard
for about 5 miles to downtown Tiburon.
From the South
Take Highway 101 North across the Golden Gate
Bridge to the Tiburon Exit.
Turn right & continue East on Tiburon Boulevard
for about 5 miles to downtown Tiburon.
We are located just past
the Beach Road intersection
on the right hand side in the Maritime Center,
upstairs over Dave & Mike’s Café.
